We are looking for
someone who is empathetic, insightful, and passionate about improving services for children with complex care needs. Previous paediatric experience, particularly with children who have complex needs, is essential.
1. Lead and coordinate the complex care discharge service, ensuring high-quality care for inpatients, including clear record-keeping and effective case load management systems.
2. Develop the service in collaboration with healthcare professionals, patients, carers, and community teams, ensuring equitable access to acute hospital care for children with complex care.
3. Provide expert advice and support to hospital staff, carers, and families, facilitating admission and internal transfer.
4. Support safe discharge planning and help prevent delayed discharge or avoidable readmissions, ensuring children receive the care they need in a timely and safe manner.
5. Act as a role model for junior staff and students, providing guidance, mentorship, and clinical supervision.
6. Lead and participate in projects aimed at improving care pathways and outcomes for children with complex care, including service evaluation and development.
7. Work closely with the multidisciplinary team to ensure a holistic approach to care planning to ensure safe and timely discharge that meets both clinical and emotional needs.
8. Provide insightful, empathetic communication with families, particularly in complex or sensitive situations, ensuring families feel supported and informed throughout the process.
