Job Overview
We are seeking a dedicated and experienced Lead Practitioner to oversee and coordinate care services at CM Bespoke Care Ltd. The ideal candidate will possess strong leadership qualities, excellent communication skills, and a thorough understanding of home care practices. This role involves guiding care staff, ensuring the delivery of high-quality support, and maintaining compliance with care plans and regulatory standards. The Lead Practitioner will play a vital role in fostering a compassionate environment where clients' needs are prioritised and met with professionalism.
Duties
* Lead and supervise care staff to ensure the delivery of personalised care in accordance with individual care plans.
* Ensure smooth operations and high standards of service.
* Assist clients with daily living activities, promoting independence whilst providing necessary support.
* Develop, review, and update care plans in collaboration with clients, families, and healthcare professionals.
* Ensure compliance with health and safety regulations, maintaining a safe environment for clients and staff.
* Provide guidance and mentorship to junior staff members, fostering professional development.
* Maintain accurate documentation related to domiciliary care and operational procedures.
* Communicate effectively with clients, families, external healthcare providers, and team members to ensure cohesive service delivery.
* Support the recruitment process by participating in interviews and training new team members as required.
* Utilise IT systems to record data accurately and efficiently manage client information.
Requirements
* Proven experience working within a home care environment.
* Strong communication skills in English, both verbal and written.
* Ability to develop and implement effective care plans tailored to individual needs.
* Driving licence is desirable for facilitating transport or home visits.
* Good IT literacy skills to manage electronic records and documentation efficiently.
* Excellent organisational skills with the ability to lead a team effectively under pressure.
* Compassionate approach with a genuine commitment to providing high-quality resident-centred care.
