Job Summary
Salters Meadow Centre is looking for a qualified Clinical Pharmacist with a prescribing qualification, ideally with experience working in Primary Care. We are a friendly and welcoming GP Practice in Burntwood, achieving high QOF results and maintaining a good CQC Standard.
Main duties of the job
You will work alongside the GP Partners and clinical team, delivering patient facing clinics, repeat prescription management, acute prescription requests, medication reviews, responding to queries and patient safety alerts. You will be an independent prescriber.

We have an established team of 4 GP Partners, an Advanced Nurse Practitioner, 4 Practice Nurses, an HCA, a Phlebotomist, and are supported by the PCN staff. Job Responsibilities
The post holder is a pharmacist who acts within their professional boundaries, supporting and working alongside a team of GPs in general practice. They will be supported by the Clinical Team, who will develop, manage, and mentor them.
The post holder will work as part of a multi‑disciplinary team in a patient facing role, taking responsibility for areas of chronic disease management and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy.
They will provide primary support to general practice staff with regards to prescription and medication queries, help support the repeat prescriptions system, deal with acute prescription requests, and perform medicines reconciliation on transfer of care. They will provide expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice.
They will provide clinical leadership on medicines optimisation and quality improvement, managing aspects of the Quality and Outcomes Framework and enhanced services.
They will ensure the practice integrates with community and hospital pharmacy to utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies and requires motivation and passion to deliver excellent service within general practice.
