You will work alongside multidisciplinary teams to create personalized care plans and ensure they are maintained in line with professional and company guidelines. * Client Communication: Maintain strong communication with residents, families, and healthcare professionals, ensuring all stakeholders are informed about the care and progress of residents. * Administration & Records: Accurately document care plans, medication administration, and other essential records. You will be responsible for reviewing care plans on a monthly basis or when changes occur. * Leadership & Support: Provide guidance and supervision to junior staff and new team members, assisting them in understanding their duties and offering ongoing support and training. * Health & Safety: Ensure a safe and secure environment for both residents and staff, adhering to company policies, and following infection control and NMC guidelines.
