Overview:
As a Community Support Worker, you will empower older individuals to manage evolving
care needs, supporting them and their families to adapt effectively at home. This includes
promoting independence, facilitating access to community-based resources, and
delivering tailored support that fosters active, connected, and fulfilling lives.
You will provide holistic, person-centred support, education, and signposting to help older
people, and their carers manage physical and/or mental health conditions while remaining
safely in their own homes for as long as possible.
This role combines home visits, telephone and online engagement, and face-to-face
community-based support. You will be part of the Service Delivery Team and report directly
to the Service Delivery Lead.

Duties and responsibilities:
* Undertake person-centred assessments to identify individual needs, strengths, and personal outcomes in partnership with clients and their support networks.
* Conduct triage and risk assessments to ensure safe, timely, and proportionate interventions.
* Co-produce tailored support plans that may involve collaboration with statutory services, third-sector organisations, and informal carers.
* Act as an advocate and liaison, coordinating care pathways and addressing barriers to access where needed.
* Deliver professional companionship and wellbeing monitoring for individuals with limited support networks, adjusting support plans as circumstances change.
* Facilitate in-person or online wellbeing sessions, including prescribed physical or cognitive recovery activities.
* Provide basic home safety awareness to individuals and carers. Support or deliver specialist workshops on dementia awareness, cognitive stimulation therapy, and related topics.
* Maintain clear, accurate, and confidential records in line with internal protocols, GDPR, and data protection standards.
* Stay informed of local health, social care, and voluntary sector services to ensure clients are connected with appropriate and timely support.
* Assist with reporting and evaluation through data analysis, case studies, and feedback gathering.
