Main responsibilities will include: Leading on supporting the childrens Community nursing team to meet the needs of children within Special School Setting. Leading on strengthening relationships and collaborative working with partner agencies to ensure children with complexity have their needs understood and safely met within their chosen setting Leading on nurse led clinic initiatives working closely with Community paediatricians supporting school and community clinics and reviews. Responsible for EHCP coordination within the CCN team. Ensuring quality person centred health advice and appropriate identification of individual need identified Responsible lead clinician for the caseload of children and young people with complex and/or additional needs within CCN team To ensure appropriate support needed is identified for children and families who are commencing nursery /school assisting in the identification of the childs health needs and how they can be met within their setting Contribute to influencing all aspects of care and management of children within the speciality. Supporting and promoting the delivery of high quality, clinically effective care, prioritising and utilising research in everyday practice. Leading on the co-ordination, development and delivery of quality training programmes to staff involved in nursery and schools responsible in meeting the health needs of a child in their care. Participation in identifying service development needs and delivering service development projects. Coordination of care supporting hospital to home and working with the childrens community team to coordinate care to the point of discharge home for children with medical complexity Work autonomously providing support and advices for families/carers who are often in hospital for long periods of time. Support in the education of CYP and their families / carers to learn new skills to meet complex health need Educate and support professionals to meet the needs of children with complex health needs Support in the development of detailed care plan and escalation care plans to support inclusion into nursery and school settings Provide clinical expertise to improve safe standards of care by active involvement in the patient journey and development of patient pathways. Work collaboratively with all stakeholders to facilitate improved patient flow, capacity and continuity of care. Support reducing length of hospital stay by assisting to streamline the patient hospital journey ie early identification of home equipment supplies /supporting appropriate orders of home oxygen. Coordination of discharge planning meetings locally, Attendance at complex tertiary discharge meetings and subsequently leading on required local planning Prevention of hospital admission providing high level community care support and review Close collaboration with Kentown lead nurse. Regular planned review of shared case load. Support for families/carers physical, emotional and social needs. Support for children preparing for adulthood and supporting transition to adult services. Developing utilisation of transition guidance /support tools. Development of transition clinic reviews, implementation and embedding of transition process and policy within the CCN team. Pivotal link between children and adult services, arranging required multi agency meetings and patient reviews Support the Childrens Community team in the development and promotion of healthcare passports. Provide highly specialist support to complex families and work with them to navigate the different systems and improve their quality-of-service experiences. Provide an integral role in leading and shaping the development and direction of the service utilising current research/evidence including NICE guidance. The post holder will be flexible with their hours to provide support as needed, which may include occasional evenings and weekends, and will work across Blackpool and Fylde & Wyre. The post holder will work within and adhere to Professional standards of practice, along with Trust policies and procedures
