As part of the duty team, you'll be the first point of contact for individuals and families seeking support, providing timely and effective interventions to ensure their safety, wellbeing, and independence.

Responsibilities:

* Conduct prompt, person-centered assessments for adults referred to the Duty Team.
* Work collaboratively with multi-disciplinary teams, including health professionals, housing services, and voluntary organisations.
* Develop and implement care and support plans tailored to individual needs.
* Manage crisis interventions, ensuring the safety and well-being of service users.
* Safeguard adults at risk by identifying and addressing concerns, adhering to national and local safeguarding protocols.
* Maintain accurate, up-to-date case records and reports in compliance with legal and organisational standards.
* Provide advice and guidance to individuals, families, and carers.
* Contribute to service development and promote best practices within the team

Qualifications and registrations:

* Qualified Social Worker
* SSSC registered in the Social Work part of the register

Skills:

* Car driver
