An exciting opportunity has arisen within Caithness Integrated Teams for a Health and Social Care Coordinator (HSCC).
Responsibilities
* Gather referral details from clients, family members, and other Health and Social Care Professionals.
* Share information with the multi-disciplinary team to prioritise and allocate resources.
* Provide support to professional staff when dealing with urgent and critical cases in the community.
Qualifications & Skills
* Excellent communication skills.
* Good time management and organisational skills.
* Flexible approach to work.
* Willingness to travel effectively and efficiently.
* Fluency in Gaelic preferred.
Employment Details
* Part‑time permanent post of 28.8 hours per week.
* Hours and hourly rate will be adjusted in line with the 2026 Agenda for Change changes.
* Travel required.
Eligibility
This role does not meet the eligibility criteria for a Certificate of Sponsorship (CoS) and NHS Highland is unable to provide candidates with a CoS.
