We are now seeking an exceptional Lead Officer to join our Social Care Connect Team, offering a pivotal opportunity to influence and improve adult social care at both an operational and strategic level. This is a rewarding leadership role within a fast paced, forward thinking service that supports adults aged 18 and over through a wide range of responsive and personalised interventions.
The role includes close collaboration with the Integrated Discharge Team, supporting people to leave hospital safely and without delay. You will play a vital role in ensuring continuity of care, working in partnership with people, carers, families and multi disciplinary professionals to secure long term, sustainable support solutions.
We are looking for a qualified Social Worker, registered with Social Work England, who brings strong leadership capability and a proactive, solution focused approach. You will have experience of leading and supervising teams, along with a sound understanding of the wider Adults and Health system. Holding the Practice Educator Award and/or Best Interest Assessor qualification is desirable, though we welcome candidates committed to professional development and willing to work towards these.
As Lead Officer, you will provide confident operational leadership, setting clear direction and supporting frontline staff to deliver consistently high quality, person centred practice. You will manage resources effectively, balancing quality, efficiency and statutory responsibilities while maintaining a strong focus on outcomes for people.
You will be highly motivated, capable of working autonomously within a complex environment and adept at prioritising competing demands. A strong working knowledge of adult social care legislation, policy and best practice frameworks is essential, as is the ability to travel independently across North Lincolnshire.
We are seeking someone who thrives under pressure, demonstrates sound judgement and can think strategically while responding decisively to immediate needs. Excellent communication, negotiation, organisational and IT skills are vital, alongside resilience, adaptability and a genuine commitment to collaborative working.
