We are seeking an experienced Care Coordinator to join our dedicated team on a locum basis, supporting our domiciliary care and supported living services across Nottingham.
This is a key role, coordinating care delivery, managing staff rotas, and ensuring that every service user receives the highest standard of person-centred care.
Key Responsibilities:
* Allocate and schedule care staff based on client needs
* Monitor care delivery and complete spot checks and audits
* Update care plans, risk assessments, and documentation
* Support with onboarding and staff performance monitoring
* Liaise with service users, families, and professionals to ensure satisfaction
Requirements:
Previous experience in care coordination or schedulingKnowledge of CQC standards and person-centred careStrong organisational and communication skillsDriving licence and vehicle preferred
