To diagnose, treat and manage patients with dermatological conditions referred to the Community Skin Health Service by GPs in accordance with the guidance from the Skin Health Clinical Lead. This will include diagnosis of a wide range of dermatology conditions and initiating medical and surgical treatment in a community clinic. To be part of the team triaging referrals received into the Skin Health Service Main duties of the job • Undertake new and follow up consultations including a full assessment of the medical condition, the patients concerns and expectations. The post holder will enter full details of history, examination findings, diagnosis and treatment plan in the clinical record on SystmOne (Patient Recording System). About us Hertfordshire Community NHS Trust offers the following: • an extensive training and education programme • a range of personal and professional development opportunities • 27 days holiday rising to 33 days (depending on NHS Trust service) plus bank holidays (pro rata where applicable) • Special Leave for family and personal reasons • NHS Car Lease Scheme • Eye Care Vouchers • Employee Assistance Programme • NHS Discounts Job responsibilities • To follow care pathways for the common skin problems as per HCT agreed guidelines. • Prescribe treatment for the presenting complaint, in accordance with the dermatology formulary and prescribing restrictions, using electronic prescribing when possible. • Offer only those treatments or surgical procedures, which are within the scope of the contract between the HCT Skin Health Service and the ICS, taking care not to undertake procedures, which are excluded from the contract. • To ensure that patient or carers are informed fully about the condition, answer any questions, supply written information if available and involve them in decisions about the treatment. • To document notes within the electronic patient record SystmOne. • Create a letter to the patients GP at the time of the consultation for every patient in accordance with Skin Health Service procedure, using the integrated word templates on SystmOne • Discuss or refer a patient for an opinion with the Clinical Lead within the community service, as required, avoiding practice outside his/her level of competence. • Undertake appropriate surgical procedures. • Order and review appropriate investigations, making sure that histology & pathology requests are correctly completed with full clinical details. • Be part of the team who triage the referrals received into the Skin Health Service. • Work with other members of staff to deliver a high quality, patient centred service which will include attendance at clinical /operational meetings and educational meetings. • Participate in the Skin Health Service clinical audit and evaluation of clinical outcomes. Share any clinical audit results with HCT. Participate in clinical governance, including attending MDTs and operational meetings • Participate in data collection including patient surveys and HCT audits as requested. • Comply with GMC standards of Good Medical Practice and the NHS constitution. • Participate in the clinical supervision programme within the Skin Health Service. This includes working with the Clinical Lead to identify gaps and CPD requirements. • To undertake all required speciality doctor training and development to maintain skills. • Clinics are provided from a variety of HCT venues in East and North Hertfordshire There is no on call commitment.
