When you think of Palliative Care, you may think of providing direct care to patients. This is very important but equally important are the administrative staff that directly support this work.

Our Palliative Care Support Officers work with Hospices, Hospitals, Community staff, care providers and Nursing Homes to make sure our patients get the care they need when they need it.

The Palliative Care Coordination Centre needs people with excellent administrative, communication skills and the ability to think on their feet to support patients and their families to stay at home or get home if they are in hospital.

We are expanding and developing the team and this is an ideal time to be part of and influence the development of palliative care within Staffordshire.

The post will be full time (37.5 hours) per week working 5 days over 7 days, 365 days a year. but part time hours are possible

Our team is based at Bradwell Hospital alongside our other services.

The post holder will be an individual experienced in administrative duties, comfortable and effective in working with service users and partner organisations.

This role will support and encourage teams to provide high quality seamless palliative and end of life care provision. The post holder will support in the coordination of care for service users and service providers across Staffordshire. This will include leading from an administrative capacity on the coordination of the provision of care in the community or care home processing applications for The End-of-Life Home Care Pathway under the supervision of the clinical team for clinical matters.

This will also include supporting with the coordination of the patient journey through the end of life to death and beyond by helping to ensure the correct elements are in place to support in the delivery of the patient’s wishes by all appropriate means.

To be the main point of contact for the multi-disciplinary team for enquiries and referrals, filtering telephone calls or redirecting from members of the public or other agencies. Accurately record messages using electronic systems ensuring effective communication within the team, organisations and partners ensuring they are dealt with safely and effectively and disseminating work as necessary.


Person specification

* GCSE English and Maths grade C or above or equivalent experience.
* IT qualification equivalent, i.e. ECDL/CLAIT or equivalent work experience to the competency level of ECDL (base profile)
* Level 3 Diploma/NVQ in a relevant subject matter such as customer service/business administration or equivalent knowledge gained through experience
* Experience of using computer based information systems in an office environment
* Understanding of the Palliative Care Coordination Centre
