Role Overview
The Children’s Continuing Care Team at GWH provides direct clinical care and support to named children and young people with complex medical needs in the community. As a healthcare professional you will deliver skilled, high‑quality care in home or community settings.
Key Responsibilities
* Carry out personal care and delegated clinical tasks for children with complex health needs, including those dependent on technology such as ventilators, feeding tubes and oxygen.
* Follow individualised care plans developed by the nursing team; achieve competency through an intensive, ongoing training programme with support from registered nurses.
* Respond to children’s health needs in their homes (including night visits) and at school, enabling families to rest and children to attend education.
* Work professionally as a lone worker in a community setting after completion of competency‑based training.
Qualifications and Skills
* Ability to work independently as a lone worker in a community environment.
* Strong communication skills and a caring, empathetic approach.
* Flexibility, reliability, enthusiasm and a willingness to learn.
Working Hours
Full‑time, part‑time or term‑time hours are negotiable for the right candidate.
