WEB Primary Care Network (PCN), comprising Primary Care Practices in Woodbury, Exmouth and Budleigh, are looking for a Health and Wellbeing Coach to join our established team. You will be expected to work at various locations across the PCN as appropriate.


Main duties of the job

Working as part of the PCN's multi-disciplinary team, the post holder will support our GP Practice patients to take steps to improve how they manage their health conditions around what matters to them. Using coaching skills, patients will be supported to address existing issues and promote lifestyle medicine to develop skills, knowledge, and confidence to manage and improve their health outcomes, wellbeing, and quality of life.

As an integral part of the PCN, the postholder will use coaching techniques to provide one-to-one advice and coaching to support patients, identifying their needs and setting goals to achieve their personalised health and care plan objectives. The role is varied and allows the Health and Wellbeing Coach to meet with patients in their GP Practice either face-to-face, on the telephone, or via video calling.

Job responsibilities

The Coach will work closely with the other members of the Primary Care Team in the management and decision-making of care and service provision for individual patients with low to medium complexity, usually with one or more long-term conditions. This includes:

* Building relationships with the PCN's multi-disciplinary team and GP Practice staff, promoting health coaching and its role in supported self-management of personalized care, actively participating in meetings, and addressing health inequalities in the PCN's population.
* Accepting referrals from the GP Practices, managing and prioritizing a caseload based on health and wellbeing needs, using a non-judgemental approach with strong communication and negotiation skills. Referring back to other health professionals within the PCN as appropriate.
* Working alongside the multi-disciplinary General Practice teams within the PCN to plan and implement a high-quality Health and Wellbeing Coaching service, supporting individuals to be active participants in their healthcare, and empowering them to manage their health and wellbeing, including:
o Engaging and motivating patients through multiple sessions to identify needs and set goals, supporting them to implement personalized health and wellbeing objectives focused on what matters to them.
o Providing interventions such as self-management education and coping skills.
o Supporting personal choice and positive risk-taking while ensuring patients understand the accountability of their actions, encouraging proactive illness prevention.
o Working in partnership with social prescribing services to connect patients to community-based activities that support them in taking control of their health and wellbeing.
o Increasing patient motivation to self-manage and adopt healthy behaviors.

Supporting one-to-one sessions on topics such as healthy eating, physical activity, and general health and wellbeing to support personalized care plans. Working within a multidisciplinary team and with local VCSE organizations. Supporting patients to manage their physical and mental health and build community resilience. Playing an active role in MDT meetings, gathering information, and updating the team on patient progress. Building and maintaining links with the voluntary sector, supporting network and partnership development. Supporting the delivery of self-care support plans for long-term conditions like COPD, diabetes, and asthma. Understanding barriers to community support and developing community-based solutions. Organizing awareness-raising events to improve community health and wellbeing. Raising awareness of available support, groups, and opportunities to help patients achieve their health goals.


Person Specification


Experience

* Experience with coaching frameworks or models, e.g., Motivational Interviewing.
* Experience working with patients on nutrition and health.
* Supporting people, families, and carers in related roles.
* Delivering group programs or support.
* Working with the VCSE sector, volunteers, and community groups.


Qualifications

* Degree in Nutrition, Exercise, Health, or equivalent.
* Willingness to attend training for a non-clinical Supported Self-Management health coaching program (minimum four days) by a PCI-accredited trainer.
* Coaching, counselling, psychology qualifications, or relevant experience involving reflective listening skills. Training in non-clinical SSM Health Coaching from a PCI-accredited organization.

* Proficiency in Microsoft Office and web services.
* Knowledge of lifestyle or health interventions, e.g., exercise and nutrition.
* Commitment to professional and personal development.
* Understanding of the personalized care approach and wider determinants of health.
* Knowledge of long-term conditions and NHS primary care.
* Experience with SystmOne clinical system.
* Facilitation skills, including conflict resolution.
