Job Summary:
We arelooking for an enthusiastic and proactive Clinical Pharmacist to enhanceour clinical team. You will work closely with GPs, nurses and other health careprofessionals to improve medicines optimisation, provide structured medicationreviews, and support safer, evidence-based prescribing.

Main duties of the job
Summary of Duties
Deliver structured medication reviews, particularly for patients with long-term conditions or on multiple medications.
Ensure safe, effective, and cost-efficient prescribing, including monitoring high-risk medicines and managing repeat prescriptions.
Conduct patient consultations (in person or remotely) and support deprescribing where appropriate.
If an independent prescriber, manage minor ailments and chronic conditions within scope of practice.
Lead on medicines optimisation, audits, QOF, and prescribing incentive schemes.
Collaborate with community pharmacies, care homes, and NHS teams to ensure seamless care.
Provide education and training to staff, keep up to date with guidance, and mentor pharmacy team members when needed.

Job responsibilities
Key Responsibilities:
Clinical Care:
* Conduct structuredmedication reviews, particularly for patients with long-term conditions orpolypharmacy.
* Support safe and effectiveprescribing, including monitoring high-risk medicines and managing repeatprescriptions.
* Provide patientconsultations (face-to-face, telephone, or video) on medicines-relatedqueries.
* Work with clinicians toreduce medication-related harm and promote deprescribing whereappropriate.
* Prescribe within clinicalcompetence and monitor treatment effectiveness.
* Manage minor conditions orchronic disease reviews under agreed protocols.
* Lead on audits, QOF, IIF,and local prescribing incentive schemes.
* Liaise with communitypharmacies, care homes, and local NHS teams.
* Advise on cost-effectiveprescribing in line with local formulary and NICE guidance.
* Promote adherence andeducate patients on safe medication use.
Education & Development:
* Provide training or updatesfor practice staff on medicines and prescribing topics.
* Stay up to date withclinical guidance and attend PCN/CPPE pharmacist events.
* Support the mentoring of pharmacytechnicians, if appropriate.
What Were Looking For:
* GPhC-registered pharmacist
* Strong clinical knowledge,communication skills, and patient focus
* Experience in primary care,hospital, or community settings
* Commitment to CPD andevidence-based practice
* Completed the CPPE PrimaryCare Pharmacy Education Pathway
* Experience using SystmOneand e-prescribing tools
* Familiarity with QOF, IIF,or other NHS England frameworks
