Job summary
An exciting opportunity has arisen for an enthusiastic and experienced AHP to join the Homefirst team in North. This position is open to Physiotherapists who have experience working in the community and wish to develop their assessment, care planning, and reablement skills whilst being part of a dynamic, progressive, integrated multiagency, multidisciplinary team.
The Homefirst team is part of the integrated community team in the North comprising registered and non-registered nursing, therapy, and administrative staff working collaboratively with partner organizations to support the intermediate care needs of our local population.
Main duties of the job
1. You will be responsible for assessment, planning, and implementation of care, assisting in the interpretation of information relevant to the patient's condition/progress, identifying problems and needs of the patient, and setting realistic goals.
2. You will need excellent organisational skills combined with a wide knowledge base and experience of working in older person's services.
3. We will offer a supportive working environment to develop your clinical and interpersonal skills, including a structured in-service training program, appraisal, and supervision system. CPD is actively supported through regular supervision and staff training.
4. The team is based at Carew House, Bodmin, covering north Cornwall from the Devon border down the north coast through Bodmin and Lostwithiel to Fowey, Par, and St Blazey on the south coast.

Person Specification
Essential:
* Degree or diploma in Occupational Therapy, Physiotherapy, or Nursing
* HCPC registered, NMC registered
* Evidence of postgraduate courses and CPD supporting practice, at least to degree level
* Substantial post-registration clinical experience in a similar role
Desirable:
* Teaching, assessing, coaching, and mentoring skills
Additional skills:
* Excellent clinical skills
* Strong verbal and written communication skills across various settings
* Understanding of clinical effectiveness and evidence-based care
