Child Disability and Transition Team
Our Child Disability and Transition team is a frontline assessment and case management team specialising in providing support and services to disabled children, young people and adults across the County Borough of Bridgend. This includes children who are care experienced and disabled young people transitioning into adulthood.
This is an excellent opportunity to join our busy, motivated and supportive team. You will have a recognised social work qualification along with knowledge and understanding of the complex needs of disabled children/young people/adults and their families.
Responsibilities
* Respond to children and families with complex needs or early child protection concerns.
* Provide high‑quality support and intervention for those with longer‑term needs.
* Deliver a Social Work service, including assessment of needs and eligible care and support plans that promote the independence and wellbeing of individuals and their families.
* Case‑manage both children and adult cases, ages 0‑25, gaining a wide range of experience across the age span.
Qualifications and Requirements
* Recognised social work qualification.
* Experience working as a social worker in children or adult services setting.
* Desire to specialise in working with disabled children/young people/adults.
* Enhanced Children and Adults Barred list criminal records check by the Disclosure & Barring Service (DBS).
* Valid driving licence.
* Commitment to protecting children, young people or adults at risk (core responsibility of all council employees).
* Knowledge that the council's Hybrid Working Policy applies to this post, establishing how working hours will be undertaken between home and office.
