Make a Difference as an Intermediate Care Support Worker (ICSW) in our Reablement Team.

We seek Intermediate Care Support Workers who enjoy working with adults aged 18 and over. The service aims to support vulnerable adults to remain as independent as possible, safely in their own homes.
You should be flexible, proactive, and capable of implementing care and rehabilitation plans in collaboration with professional staff. We are launching a new partnership with the Whiston Hospital Discharge team called Home First, intended to expedite safe hospital discharges.
The successful candidate will demonstrate the ability to support colleagues, work with electronic care systems, engage with health and social care professionals, and support service users in the community, helping to prevent hospital admissions and facilitate timely discharges.
Our office is based at Nightingale House, adjacent to Whiston Hospital. Our service operates from 07:30 to 22:00, 365 days a year.
You will work 22.5 hours on a 10-week rolling rota, including early and late shifts, or 26 hours on an 8-week rota, including early and late shifts.
We will provide training and support to ensure your success in this role.
