Job Summary
Dynamic, confident and motivated Mental Health Nurse, Occupational Therapist or Social Worker with excellent communication skills and the ability to assess and lead on clinical decision making. An exciting role within a fast‑paced, multi‑disciplinary team providing high‑quality assessment and home treatment services as an alternative to hospital admission for individuals experiencing acute mental health crisis.
Key Responsibilities
* Provide first‑line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
* Conduct rapid assessments of mental health patients who are vulnerable to hospital admission.
* Deliver specialist crisis assessments with a focus on 'hospital at home'.
* Co‑ordinate the Crisis and Home Treatment Team on a shift‑by‑shift basis, acting as Duty Worker to triage referrals, plan shift activities and organise the team accordingly.
* Participate in team MDMs (multi‑disciplinary meetings).
Person Specification – Essential
* Previous and relevant post‑registration experience as a Mental Health Nurse, Occupational Therapist or Social Worker.
* Qualified and current registration with the relevant professional body (NMC, HCPC, Social Work England).
* Ability to meet the travel requirements of the post.
* Successful Disclosure and Barring Service check (subject to the Rehabilitation of Offenders Act).
