Role
We are looking for an individual to work as a Band 3 Discharge Co‑ordinator within the Coastal Lodge Inpatient Rehabilitation Team to cover maternity leave. The role involves working with inpatients, their next of kin and multidisciplinary teams to support and facilitate early discharge planning and avoid delays.
Key Responsibilities
* Build a deeper understanding of patient needs by engaging in conversation soon after admission regarding home environment and family circumstances.
* Identify the right expertise and seek innovative solutions to meet patient needs.
* Clearly set expectations around the discharge journey for patients and their relatives.
* Attend morning handover, MDT meetings and meet with next‑of‑kin as required.
* Work alongside the MDT to ensure effective admission and discharge processes.
* Communicate effectively with MDT, social services, cluster leads, SPA and voluntary agencies regarding simple and complex needs identified during admission to enable timely discharge.
* Ensure all appropriate literature is disseminated and understood by patients and next‑of‑kin to set expectations and highlight concerns early.
Contract Details
This is a 20‑hour, fixed‑term contract (subject to secondment if you are currently employed by the NHS). The role is flexible over the week but requires you to be available for morning handover, MDT meetings and other times agreed with the Coastal Lodge team. Applicants requiring UK Skilled Worker sponsorship are ineligible to apply.
Requirements
Applicants must obtain an Enhanced Disclosure and Barring Service (DBS) check; the Trust will cover the cost of the check. Applicants who subscribe to the DBS update service may present a valid DBS certificate.
