Your role and responsibilities
This is an exciting opportunity to work as an Independence and Wellbeing Practitioner (IWP). The role will require you to represent Adult Social Care and work alongside the people of Suffolk, their families, and advocates as well as other professionals and agencies. You will be reviewing people in hospital and facilitating their hospital discharges.
You will work as part of the Adult Social Care hospital team using the discharge to assess model alongside our health partners in Ipswich Hospital.
You will:
5. work closely with Social Workers, other IWP's, and staff from across the locality to seek the best outcomes for people
6. be involved with using Digital care solutions to assess and support people
7. actively contribute to the ongoing development of the service
8. have the opportunity to progress your career, including through applying to the social work apprenticeship scheme or occupational therapy apprenticeship scheme, when available.
You will need
9. excellent communication skills to engage with the people we are working alongside, their families and/or advocates as well as other professionals
10. enthusiasm to contribute to a multi-agency team approach for responding within urgent timescales
11. resilience under pressure and the ability to remain positive when challenged
12. to work one Saturday a month.

The team seeks to improve the quality of life and independence of the people we work with. Within the hospital we continue to develop changes to discharge processes. These will minimise a person's stay in hospital whilst maximising the opportunities our teams have to work with people in a community setting focussing on reablement and improving their health and long-term wellbeing.
