Role Overview
Essex Partnership University Trust (EPUT) is recruiting Health Care Assistants (HCAs) for our community rehabilitation service. The role is part of the Collaborative Care Team, which works jointly with the Frailty Virtual Ward and the Community Stroke Team / Early Support Discharge Service.
Team & Services
The Collaborative Care Team is a multi‑disciplinary, therapy‑led team providing rapid assessment and rehabilitation for patients in their own homes. It supports the facilitation of discharge and the prevention of admission for patients requiring rehabilitation for conditions such as stroke, orthopaedics, unstable fracture management, and neurological issues.
Responsibilities
As part of a friendly supportive team, you will provide person‑centred care and support recovery. You will assist in the provision of rehabilitation programmes during a range of shift patterns over seven days per week, in line with service needs. You should be motivated, willing to learn, enthusiastic and demonstrate good interpersonal skills when communicating with patients, families, carers and colleagues.

Induction Programme
The Mid and South Essex Integrated Care System (ICS) has launched an HCA Academy, an initiative between EPUT and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming HCA workforce. You will attend a 5‑day, face‑to‑face induction programme at the MSEICS Education Hub at Brentwood Community Hospital. Your start date will align with one of the two induction dates discussed during recruitment.
