Overview
Lincolnshire Partnership NHS Foundation Trust (LPFT) offers a range of services to support people with learning disabilities in the community. LPFT’s adult learning disability service provides specialist health support to people with learning disabilities who require assessment and/or treatment for their physical and mental health, including support with behaviours of concern. The service operates from three multi‑disciplinary hubs across Lincolnshire and delivers specialist provisions as well as a county‑wide community home assessment and treatment team.
Responsibilities
* Provide clinical care across the east of Lincolnshire (Boston, Mablethorpe, Skegness, etc.)
* Serve as liaison support for people with learning disabilities, including validating GP registers and delivering training
* Cover annual leave and sickness of colleagues and, at times, support a county‑wide role
* Promote the rights of people with learning disabilities, ensuring reasonable adjustments, applying the principles of the Mental Capacity Act, and training GP staff
* Facilitate acute liaison when required and build positive working relationships with partner agencies
* Maintain a high standard of clinical practice in line with professional code of conduct and support continuous quality improvement
Qualifications and Skills
* Registered practitioner, band 5–6 progression post
* Enthusiastic, motivated, and able to provide safe, effective, evidence‑based care to people with learning disabilities
* Strong initiative, excellent teamwork ability, and willingness to undertake relevant training
* Excellent interpersonal and communication skills, with the ability to develop and motivate others and demonstrate a progressive mindset
* Commitment to high‑quality care and continuous quality improvement
