Key Responsibilities:
* Provide expert Social Work and Care Management services to residents and carers in Hounslow.
* Conduct person-centred Social Care Assessments, focusing on individual needs and desired outcomes.
* Safeguard the rights of residents and carers, ensuring a safe environment in line with the Council’s and Pan London Safeguarding Policy.
