Job summary
The post is a replacement 10 PA post. Sedgefield treatment and intervention team is based in Spennymoor. The team provides secondary mental health service input for all mental health conditions for the working age adult population. The team receives referrals mainly from GP surgeries via the central Access team and other sources such as Crisis team/ Liaison Psychiatry/wards. The team offers specialist support to patients with significant mental health difficulties such as mood disorders, schizophrenia, bipolar disorder, personality disorder, and adult neurodevelopmental conditions like ADHD and Autism. The team can access expert advice from Trust-wide specialist services including ADHD, Autism Spectrum Assessment Team, and eating disorder services. The Crisis Team manages access to inpatient services. The post-holder will develop effective working relationships with the Crisis and Inpatient Teams by liaising closely when patients are referred to the crisis team or require alternative to hospital beds, which will be managed by the crisis team, while patients needing hospital care will be managed by the inpatient team. The post will join a dedicated multidisciplinary team, contributing to compassionate, evidence-based patient care and service improvement.
Located in Spennymoor, County Durham, working into Sedgefield North PCN which covers 10 GP Practice groups in the area.
Main duties of the job
The Consultant will work within a multidisciplinary team, providing clinical expertise for both new and ongoing cases, especially around psychopharmacology, diagnosis, risk assessments, and liaising with local GPs and inpatient units. All inpatient referrals will initially be seen by the Crisis Team based nearby at West Park Hospital, Darlington.
Most outpatient work will be at Enterprise House, with some clinics at Pioneering Care Centre, Newton Aycliffe. The role also involves home visits and outreach as needed. The consultant is expected to liaise with inpatient teams for formulation and transfer of care meetings.
