Each community nursing team forms part of an Integrated Community HUB and each team requires a team leader. Opportunities have arisen for Community Nurse Team Leads to manage a community nursing team, taking overall responsibility for the assessment, management, and evaluation of evidence-based nursing care to patients in various settings.
We have vacancies for Community Nurse Team Leads in the East North Community Nursing HUB, based at Melton Mowbray and Syston Health Centre. Part-time hours (minimum of 30/week) will be considered for the vacancy at Syston Health Centre.
It is essential that you hold a UK driving licence and have access to a vehicle for work purposes.

Main duties of the job
The posts are offered on a 12-month secondment basis, with the requirement that successful candidates apply for and secure a place on the next available District Nursing course to gain a District Nurse Specialist Practitioner qualification. If a candidate does not secure a place, the secondment will end immediately, and they will return to their previous role.
The Community Nurse Team Lead will work with Nurses for Complex Care, Senior District Nurses, and Senior Nurses for Complex Care to deliver high-quality care. Responsibilities include overseeing care quality, clinical activity, providing professional nursing leadership, guidance, clinical advice, and management.
