Responsibilities
* Support the Home First team with customer assessments and reviews.
* Work with allocated customers to identify and deliver high-quality, person-centred outcomes.
* Build strong relationships with customers to achieve personalised goals and improve independence.
* Work collaboratively with social work teams, health partners and the voluntary sector to support integrated care.
* Make a meaningful difference to the lives of residents across Suffolk.
* Receive regular support and professional supervision as part of a collaborative team environment.
