Overview
Are you the nurse we are looking for to join our team? As a Practice Nurse you will join the primary care team at Tywyn Health Centre, contributing to delivery of a new model of multidisciplinary health care services for Tywyn and surrounding areas. The team includes Advanced Nurse Practitioner, Primary Care Practitioners, Practice Nurses, Advanced Pharmacists, Physiotherapist, GPs and other allied health professionals.
You will undertake a wide range of nursing duties relevant to primary care, including wound care, immunisation, contraception and sexual health services, cervical screening, warfarin monitoring, lifestyle advice, assisting with minor surgery, travel health and long-term condition management. Experience of long-term conditions and working in General Practice is preferred.
English and/or Welsh speakers are welcome; Welsh language ability is desirable.
Responsibilities
* Perform tasks prescribed and delegated by GPs and Senior Nurses, working within Patient Group Directions, agreed protocols, and defined competencies.
* undertake chronic disease management clinics to support QoF (Asthma, COPD/Spirometry, Diabetes, Chronic Heart Disease).
* Identify and manage treatment plans for patients at risk of developing long-term conditions.
* Perform basic nursing tasks: ear care, blood pressure monitoring, ECGs, wound assessment and care, including removal of sutures/clips and leg ulcer management.
* Participate in vaccination and immunisation programmes; administer injections and ensure safe vaccine storage and cold chain policy adherence.
* Undertake cervical cytology screening as eligible, and conduct health screening, surveillance and new-patient screens.
* Take pathology samples (blood, urine, etc.) for analysis; assist GPs with minor surgery procedures.
* Assess problems presented by patients, manage minor illnesses, and contribute to minor illness clinics.
Qualifications
Essential Criteria
* Registered first level nurse with active statutory registration
* Previous GP Practice Nurse experience
* Knowledge of care for patients with long-term conditions
* Aware of accountability of own role and other roles in a nurse-led service
* Knowledge of health promotion strategies
* Knowledge of clinical governance issues in primary care
* Knowledge of patient group directions and associated policy
* Good communication skills; able to work in a team and with patients and families
Desirable Criteria
* Relevant nursing degree
* Membership of a professional body
* Some community or primary care nursing experience
* Experience of nursing long-term conditions
* Ability to identify determinants of health in the local area
* Knowledge of public health issues in the local area
* Awareness of local and national health policy and the wider health economy
Other Requirements
* Ability to work core hours with a flexible approach
* Welsh language ability (desirable)
