If so, we have a fantastic opportunity for an experienced Nurse Advanced Clinical Practitioner to bring their expertise and knowledge to assist in the development, implementation, and ongoing delivery of clinical care as part of the Integrated Virtual Ward Clinical Team. The role involves working very closely with the Acute Frailty Virtual Ward multidisciplinary team (MDT) and is a pivotal role in ensuring all patients within the Frailty Virtual Ward receive the best possible care and service. This post is available for a dynamic and experienced Advanced Clinical Practitioner (ACP) with an interest in the provision of care to patients suffering from co-morbidities, frailty and in comorbid patients.
Main duties of the job
The role involves working very closely with the Acute Frailty & emergency departments as well as visiting patients in care homes and in the community, providing a range of services using your enhanced clinical assessment and treatment skills, to assess and manage patients. The virtual ward will initially be patients who have been admitted to the virtual ward following transfer from an Acute Hospital ward however will develop to accept patients with direct admission from primary care and other community‑based providers.

Person Specification
Education / Qualifications
* MSc in appropriate clinical field
* Independent/Supplementary Prescribing (V300)
* Registration with health governing body (e.g. NMC, HCPC)
* Advanced Clinical Practice qualification
* Diploma in Geriatric Medicine
Skills / Abilities
* Ability to work autonomously and organise and prioritise workload
* Ability to negotiate effectively with different disciplines and all grades of staff
* Clinical leadership and decision‑making skills
* Experience of Acute & Community working
Experience / Knowledge
* Up to date on developments in frailty management and prevention
* Experience of Quality improvement process implementation
