The post holder will work as part of the cardiac service and will provide caring support for individuals within their own homes and during community clinics, under the direction of a registered professional. The post involves working in partnership with patients, carers, and other health, social care, and voluntary agencies to promote independence. The role requires multi-professional teamwork, supporting all team members' roles, and contributing to the future development of the service. This position is for 12 hours on Thursdays and Fridays only.
Key Responsibilities:
1. Carry out care, nursing, and therapy interventions within your competency, under the guidance of a qualified registered professional.
2. Monitor patient progress, report findings to registered staff, and implement changes to management plans as directed.
3. Provide accurate information to registered nurses and therapists to assist in patient assessment and evaluation.
4. Assist Cardiac Nurses in monitoring, treatment, care, and rehabilitation.
5. Ensure timely entry of activity data.
6. Safeguard patients by taking emergency action when necessary, reporting risks immediately, and following safeguarding policies.
7. Communicate effectively with colleagues and stakeholders, using electronic communication within the framework of Information Governance policies.
8. Perform community care procedures and clinical observations, such as blood pressure monitoring, urinalysis, venepuncture, 12-lead ECG recordings, and managing medical equipment.
