What you will do:
- Coordinate and manage the hospital discharge process, ensuring a smooth transition for adults returning to their homes or community settings.
- Collaborate effectively with a range of healthcare professionals, ensuring the best outcomes for individuals and their families.
- Conduct assessments and create tailored care plans, adapting promptly to any changes in circumstance or need.
- Leverage your expert knowledge and significant frontline experience within adult services to support and guide less experienced team members.
- Maintain accurate and up-to-date records in compliance with Social Work England standards.
Requirements of the Social Worker:
- Hold a degree or equivalent qualification in social work.
- Maintain current registration with Social Work England.
- Demonstrate significant senior level frontline experience within adult services, ideally with a focus on hospital settings.
