About the Role
Families caring for children with life‑shortening and highly complex health conditions often experience chaotic, stressful, and exhausting journeys when accessing palliative care, symptom management, and practical support. Forget Me Not Children’s Hospice, a CQC‑rated outstanding provider, offers a new approach to supporting these families. This apprenticeship is part of the family support team and involves developing and embedding this new way of working, ensuring each family receives timely, culturally appropriate care.
Responsibilities
* Support a caseload of families, providing information, advice, advocacy and handholding.
* Link families to tailored support, both in‑house and by collaborating with local partners.
* Build partnerships with external services, tapping into community groups to ensure culturally appropriate and accessible support.
* Support older children, young people and families on the caseload, including those transitioning beyond hospice support.
* Deliver person‑centred, strengths‑based support informed by an understanding of palliative care and multi‑agency collaboration.
Qualifications & Experience
* Experience working with families with complex needs and circumstances.
* Background in education, health or social care acceptable; knowledge of palliative care valued.
* Experience with multi‑agency collaboration.
* Experience engaging families from diverse cultural, ethnic and faith backgrounds.
* Desirable: Ability to speak additional community languages relevant to the local population.
