An exciting opportunity has arisen for a Band 6 Community Care Coordinator to join our lively and friendly Bradford City and North Community Mental Health Team for Older People. Based in the multicultural city of Bradford, we are seeking skilled, enthusiastic, and compassionate clinicians to deliver outstanding care to our diverse community. Covering both City and North areas, we take pride in serving one of England's most culturally rich populations and continually explore innovative, inclusive, and person‑centred ways of working that celebrate diversity.
As a Community Care Coordinator, you will support clients through challenging times, empowering them toward recovery and independence. You will manage a caseload, coordinate holistic care, and deliver recovery‑focused interventions within a supportive multidisciplinary team. You will be expected to act as a role model for less experienced staff and have opportunities to be involved in service delivery improvements and innovations.
This is a fixed‑term 12‑month contract providing maternity cover. It is a Band 6 clinician role requiring a role model and resource for staff, with regular supervision and support. A full and valid driving licence is required.

Right to Work in the UK
