Position Summary
The Crawley Healthy and First Community Primary Care Network Team is looking for a registered nurse to join the team. We are based at Crawley Hospital covering Crawley and Horley.
Key Responsibilities
* Diabetic management
* Wound management, including leg ulcer diagnosis and treatment
* Palliative care
* Bladder and bowel care, including urethral and suprapubic catheterisation and other continence needs
* Working in partnership with local health and social providers, particularly with people under frailty
* Delivering high‑quality community care, supporting patients towards rehabilitation, self‑management and independence across the continuum of care
* Participating in evidence‑based care and facilitating staff learning and development

On the Person Specification there are some of the competencies and expectations you will either have or need to develop if you were successful. The main aspects highlighted are diabetic management, wound management (including leg ulcers), palliative care, bladder and bowel care, and other continence needs.
