Practice Nurse, Chronic Disease - Crewkerne, Somerset

Symphony Healthcare Services (SHS) has an exciting opportunity to join Crewkerne Health Centre in Somerset.
This is a permanent, part time post of 31.5 hours per week.
The annual salary range for this post is £39,146.36 - £41,851.01 per annum (pro rata for part time hours, depending on experience).
Main duties of the job
The main duties of a SHS Chronic Disease Practice Nurse are:
* Manage a designated case load of patients with chronic conditions, visiting them in their own home or consulting with them at the practice.
* Work within the NMC Code
* To mentor junior staff ensuring that they are supported and continually developed.
* To develop the nursing services offered both for the practice and through SHS.
* To support clinical Supervision within the practice.
* To work with the practice and Lead Nurse in the development of clinical skill mix
* Promote SHS values and acts as a role model.
* To Implement the SHS Nursing Strategy as defined by the Nursing and AHP Director and the Lead Nurses forum.
* To ensure a high level of patient involvement and satisfaction with the service, including linking in with the practice PPG.

* Registered first level nurse
* Evidence of appropriate continuing personal development
* Qualification in chronic disease management e.g. Hypertension, Mental Health, Diabetes, COPD, CHD.
* Clinical Supervision Training and experience D

* At least 2 years recent nursing experience of working with patients with chronic conditions.
