The aim of the service is to:
* Provide a continuum of family support
* Offer intensive support to families on the brink of care or where the plan involves a child's rehabilitation home
* Work with families to focus on their strengths, building resilience to enable positive changes
* Deliver evidence-based interventions tailored to the family's needs
* Help children, young people, and families identify and achieve goals
Job Role
To provide intensive family support to families at risk of becoming Looked After by the Local Authority. Support may be required during evenings and weekends. Initial responsibilities include:
* Engaging families through effective communication, building collaborative relationships, and including their views in family plans
* Gathering, analyzing, and evaluating information from children, families, and agencies to assess needs
* Assisting families in establishing goals collaboratively
* Prioritizing child protection and safeguarding needs
* Supporting families in creating safe environments
* Planning and delivering evidence-based interventions to modify harmful behaviors and reduce risks
* Helping families identify strengths and support networks for change
* Identifying appropriate resources and advocating on behalf of families
* Participating in service evaluation and development
* Preparing reports for care proceedings, Child Protection Conferences, and Care Reviews
* Building and maintaining productive relationships with other agencies and organizations
* Recording and monitoring information for service evaluation
* Ensuring smooth transfer of families between intervention phases
* Providing supervision, support, and guidance to team members to ensure high performance and professional development
