Overview
Liverpool Crisis Resolution Home Treatment (CRHT) team is recruiting a Band 6 practitioner to join the team. CRHT provides intensive home support as an alternative to hospital admission across Liverpool and Sefton and operates on a 24/7 basis. The post holder will travel between multiple sites and work flexibly to meet service demands.
Responsibilities
* Conduct comprehensive mental health assessments, including risk assessment, and formulate care plans tailored to individual needs during crisis episodes.
* Operate as part of an integrated Home Treatment Service, delivering short-term interventions and therapeutic support to prevent unnecessary hospital admissions.
* Collaborate with the Multi-Disciplinary Team, community mental health teams, social services, police, GPs and voluntary agencies.
* Work on a rotational shift basis, including weekends and night shifts, and participate in a 24-hour roster system.
* Deliver care across a variety of environments with a person-centred approach that respects service users’ choices and involves carers in care planning.
Requirements / Qualifications
* Band 6 practitioner (or equivalent) with appropriate professional qualification in mental health.
* Experience in crisis/home treatment settings is preferred.
* Ability to work collaboratively with a range of stakeholders and travel between sites as required.
* Commitment to flexible working and 24/7 service availability.
