Bedded Units Medicines Optimisation Pharmacist - Band 7
An exciting opportunity has arisen for a dynamic, motivated and innovative pharmacist to join our expanding Medicines Management team at Central London Community Healthcare Trust. We are looking for a full-time pharmacist to provide a clinical pharmacy service at our bedded units within South West Hertfordshire & North London. As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in providing and developing a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Discharge to Assess ward) covered by the CLCH Bedded Services team. You will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties and be supported by experienced Senior Clinical Pharmacists to gain new skills in each rotation and develop strategic, quality improvement and leadership skills.
Responsibilities
* Provide a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Discharge to Assess wards).
* Provide professional expertise, advice, support and training to health care professionals and patients within the Trust.
* Be responsible for individualised patient pharmaceutical care, ensuring clinical prioritisation on a day-to-day basis at ward level.
* Collaborate with the Medicines Management team and contribute to service development, quality improvement and leadership activities.
* Travel across different sites within the Trust where Bedded Services inpatients are located; hold a full, valid UK driving licence and have access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010).
Requirements / Qualifications
* Registered pharmacist eligible to practice in the UK, with current Band 7 level experience in medicines optimisation/pharmacy services.
* Full-time commitment to clinical service delivery in inpatient settings.
