Paediatric Palliative Care. Children's Community Nursing Service

Working as part of the Aneurin Bevan University Health Board Community Children's Nursing Service and the All Wales Managed Clinical Network (AWMCN), the post holder will lead the delivery of specialist palliative care and advice.

Adopting a family-centred care approach, this role involves supporting families from diagnosis through end-of-life care and bereavement, collaborating with multidisciplinary teams, and empowering staff to deliver excellent nursing care in line with evidence-based best practices.

The post holder will work with families and members of the multidisciplinary team, liaising with inpatient, community, and hospice services to coordinate and support the delivery of high-quality holistic care to babies, children, and young people with life-limiting or life-threatening conditions, by providing clinical expertise, supporting complex decision-making, advanced care planning, and symptom management.

This is an exciting time to join our friendly and supportive team and play an active role in service development, helping to inform the delivery of specialist paediatric palliative care services within Aneurin Bevan Health Board and by working in partnership with Paediatric Palliative Care Specialist Nurses from across the NHS.

Main duties of the job

The post holder will expertly assess, monitor, and coordinate a holistic, individualised, therapeutic plan of care in partnership with the child/young person, their families, and professionals. The aim of this is to empower resilience and promote quality of life, utilizing recognised assessment tools as well as expert clinical judgment. Support consultant-led paediatric palliative care clinics and run nurse-led clinics independently. Be responsible for managing a defined caseload without supervision. Lead and coordinate multidisciplinary assessments and meetings for individual children to ensure continuity of care and support from all involved services. Interpret complex patient condition assessments and multifaceted situations, comparing a range of options to determine appropriate treatment. Provide care in the community, acute, and voluntary sectors, liaising and coordinating across each. Promote best practice and challenge unsafe practices.

Work innovatively, devising novel solutions within own and others' practice to improve quality of life outcomes in an unpredictable environment. Act as a positive role model and play a prominent role in raising the profile of paediatric palliative care. Collaborate with the Community Children's Nursing Team to provide support to children, young people, and their families known to both services.

Job responsibilities

We seek a motivated, passionate, and experienced Paediatric Palliative Care Clinical Specialist Nurse to work within the Community Children's Nursing Service (CCNS), part of the Family & Therapies Division at Aneurin Bevan University Health Board.

This role involves supporting families from diagnosis through end-of-life care and bereavement, collaborating with multidisciplinary teams, and empowering staff to deliver excellent nursing care based on evidence-based practices.

The post holder will liaise with inpatient, community, and hospice services to coordinate and support high-quality holistic care for children and young people with life-limiting or life-threatening conditions, providing clinical expertise, supporting complex decision-making, advanced care planning, and symptom management.

Person Specification


Clinical Nurse Specialist - Paediatric Palliative Care

* RSCN/Child Branch Relevant Degree or equivalent knowledge, skills, and experience.
* Evidence of relevant specialist education, including a Postgraduate Diploma in Palliative Medicine.
* Further study in palliative care.
* Basic IT skills.
* Welsh language skills (desirable).

Senior Nurse, Community Children's Nursing Service

Salary: £46,840 to £53,602 per annum (pro rata).
