Location details: Portsmouth MH Serv Lot 1 - Outram provides supported accommodation for up to 12 adults with mental ill health and alcohol/substance misuse needs. Our clients have a mix of medium to high support needs. This service offers 24/7 support across two houses next door to each other. The aim is to help clients develop life skills to achieve goals, gain independence and move on to safe and secure accommodation.
The role
* Lead on the case management of clients and be part of a team that supports them in staying safe and achieving their goals.
* Provide homes and specialist support so clients feel valued, secure and ready for the next steps.
* Produce and regularly review comprehensive client-based risk assessments, engage with local authorities, health providers and other agencies, and maintain constructive working relationships to maximise opportunities for clients.
* Lead client activities to help them gain practical life skills and encourage engagement with the running of the service.
* Assist clients with finances, rent payments, arrears management and accessing benefits; provide encouragement and signpost to local community resources and opportunities (education, training, employment, volunteering).

* A full driver’s licence and access to a vehicle are required.
* Flexibility to work across several sites and to work with clients one-to-one.
* Ability to work on a six-week rota including early, middle and late shifts.

* Strong communication skills, both verbal and written
* Clear verbal and written English
* Good working knowledge of Microsoft Outlook, Excel and Word with data entry skills
* Confident and assertive manner
* Ability to self-motivate when working independently
* Effective teamwork
* Ability to respond calmly to crisis and manage challenging situations
* Genuine interest and concern for homelessness and related issues
Desirable criteria
* Experience managing a caseload of clients
* Understanding of the complex support needs of people experiencing homelessness or with complex needs
* Knowledge of voluntary and statutory agencies, housing regulations and the benefits system
* Understanding of risk assessment and person-centred, outcomes-based delivery
