Job Title: Adult Social Worker - Intermediate Care Team Location: Somerset Rate: £38.00 An exciting job role has recently become available at the Somerset. They are looking for a new member for their Intermediate Care Team. What will your responsibilities be? You will manage a caseload of varying complexity, commensurate with ability and qualifications, by receiving and responding to referrals from various sources. You will implement Community Care and an integrated care program approach, conducting risk assessments to ensure the safety and protection of individuals. Conduct statutory reviews and comply with appropriate legislation, policy, and procedures. Assess client needs in consultation with them and other professionals, agreeing on levels of support. Engage with individuals who refuse services but are at risk to themselves or others. Navigate sensitive negotiations during assessments with service users and carers where there is conflict or lack of insight. Effectively manage crises, providing quick resolutions using relevant legislation and resources.
