Conduct assessments to identify family needs and strengths. Develop individualized support plans in collaboration with families. Facilitate access to community resources and services such as housing, education, and mental health support. Monitor and evaluate family progress towards goals, adjusting plans as necessary. Offer parenting education and support to enhance family functioning. Maintain accurate and confidential case records and reports. Requirements Bachelor's degree in social work, psychology, or related field preferred. Experience working with families and children in a social service setting. Strong interpersonal and communication skills. Ability to work collaboratively within a team and with external partners. Cultural competence and sensitivity to diverse family dynamics. Excellent problem-solving skills and the ability to manage difficult situations. Familiarity with community resources and services available to families.
