* Conduct assessments to understand and support the needs of adults within the South Neighbourhood Team
* Deliver comprehensive Care Act duties to promote wellbeing and autonomy of service users

* Use your experience in Adult Social Care and Liquid Logic systems to maintain accurate records
* Coordinate and facilitate care plans tailored to individual needs
