We are looking for a Social Worker who is passionate about care delivering care to adults with mental health issues, registered with Social Work England, with experience of working in a community based setting. The role involves delivering care to an allocated caseload of clients living in the community and supporting them within a recovery framework as part of a multi‑disciplinary team.
Responsibilities
* Champion social care with a comprehensive understanding of legislation and statutory guidance.
* Take a lead in the promotion of adult safeguarding practice, undertake safeguarding enquiries, and contribute to the provision of care plans that protect adults and children from abuse.
* Undertake comprehensive assessments and reviews for people presenting high risk or with significant social, legal and environmental complexities, ensuring appropriate assessment against the Care Act 2014 criteria.
* Offer direct support to service users and carers, providing information and advice when eligibility for social care is not met.
* Ensure appropriate health and social care services are commissioned, offering direct payments and processes for charging by the local authority where applicable.
* Ensure commissioned services allow choice to service users and carers and focus on community-based support and access to universal services.
* Conduct regular risk assessments and keep vulnerable people safe.
* Conduct carer assessments, including identifying contingency plans and providing support.
* Use partnership working to identify, develop, and facilitate a wide range of recovery‑focused community services, informing colleagues of best practice.
* Engage service users and carers to learn from their experiences to improve service delivery.
* Maintain accurate computer records of all work undertaken.
* Undertake post‑qualifying and other appropriate training to keep high standards of practice and continue registration with Social Work England.
* Provide both management and reflective supervision to newly qualified social workers and unregistered social care staff.
* Support newly qualified social workers through the Assessed and Supported Year in Employment (ASYE) and provide opportunities to train as an ASYE assessor.
* Undertake duties as an Approved Mental Health Professional (AMHP) or be willing to undertake AMHP training.
* Participate in regular supervision and ensure regular reflection on personal practice as both a social worker and AMHP.
* Undertake other tasks and duties which form part of the delegated social care responsibilities accepted by the Trust.
* Adhere to Trust policies and procedures.
* Undertake additional training as required.
* Carry out any other duties as identified by the Line Manager.
Qualifications
* Appropriate professional Social Work qualification.
* Registration with Social Work England.
* Training in principles and application of outcomes, focused assessments and personalisation.
Desirable Qualifications
* Approved Mental Health Professional / Best Interests Assessment Training.
* Experience as an Approved Mental Health Professional and/or Best Interests Assessor; appropriate UK professional registration.
