Working Hours for Care Team Co-Ordinator:
Rotational shift pattern Monday to Sunday with on call responsibilities on a rotational basis
Job Description for Care Team Co-Ordinator:
As a Team Co-Ordinator, you will provide a person-centred approach that supports self-awareness, personal growth and gives each person the strongest voice with regards to decision making and lifestyle choices. As a Team Coordinator, you will be employed by Futures Care, as such Futures Care reserve the right to ask you to work at all services depending on the overall service need. As a Team Co-Ordinator, you will be a multi-tasker with excellent time management and communication skills with an upbeat attitude. You will have extensive previous experience with caring and supporting individuals who have complex and clinical needs such as Autism, Learning Difficulties, Mental Health, Acquired Brain Injury, behaviours that challenge, and Epilepsy is preferred
