What You Will Do
An exciting opportunity has arisen to become part of the Enhanced Intermediate Care Team to provide short term support for people with conditions that can be safely managed at home or in community but require additional health and social support to facilitate this. The service can facilitate early and supported discharge from hospital or provide an alternative to hospital admission. It aims to maximise the long‑term independence, choice and quality of life and as a consequence reduce the need for on‑going support from formal services.
