Location: Hounslow, based at Clayponds Hospital. Service operates 19:00 – 07:00, seven days a week (two shifts: 19:00 – 00:00 Twilight; 19:00 – 07:00 Overnight). The role is a permanent part-time position (11 hours per week).
Overview
The Senior Community Nurse provides senior nursing support within the Adult Community Nursing Service, Night Nursing, working across the locality to support high-quality patient care and service delivery.
Responsibilities
* Assist the Locality Nursing Team Manager in the delivery and management of a designated caseload, ensuring holistic assessments are undertaken by a competent practitioner within the nursing team.
* Support Locality Nursing Team Manager in line management and leadership for the community nursing team, accountable for the team caseload and the development of a skilled, competent and motivated workforce.
* Take a lead for a specific area of nursing practice to share best practice knowledge and support less experienced nurses.
* Deputise in the absence of the Locality Nursing Team Manager and undertake delegated tasks as development opportunities for succession planning.
* Work independently and as part of a multidisciplinary locality team to deliver the locality nursing service in line with the commissioned service specification, standards, activity and outcomes.
Qualifications and requirements
* Band 6 Senior Community Nurse role; evidence of senior nursing experience in community settings.
* Full UK driving license and access to a car.
* Excellent communication skills and the ability to lead and motivate a team.
Training and development
