Band 4 Cancer Clinical Support Worker

We are looking for a Band 4 Cancer Clinical Support worker. The post holder will form an integral part of the Gynae Oncology team to provide cancer patients with information and support that meets their individual needs throughout treatment and whilst living with cancer. As part of the gynae oncology team you will have a fundamental role providing patients in clinic and on the telephone with support throughout their treatment and living beyond their diagnosis including nearing end of life.
Provide cancer information and/or signpost patients and families to support services and resources. Develop a rapport with the patient and their families and being a link to the wider team, you will work closely with the clinical nurse specialists and the oncologists. Document your communications on the Somerset Cancer Registry.
Main duties of the job
Provide basic telephone/ face to face advice and triage where appropriate, working within defined protocols and guidelines. Document and communicate outcomes as per service specification.
Support patients and carers to understand what signs, symptoms or situations to be aware of that would indicate concern.
Encourage and support active lifestyle choices. Assist with the delivery and signposting of patients to Cancer Health and Wellbeing pre and post treatment programmes. Contribute to and deliver education and training to patients and carers, including information around health promotion and supporting self‑management on a regular basis.
Assist and promote cancer awareness campaigns and engagement events. Ensure patients and carers are treated with dignity and respect.
Develop and maintain good professional relationships with patients, carers and specialist cancer teams at UHB. Assist patients and relatives overcome barriers to understanding when English is not their first language.
Collaborate and work closely with local providers of validated information resources, users and user groups, UHB Cancer Information and Support Service and third sector support, e.g. Macmillan, Tumour Specific charities and Local Cancer Charitable organisations. To provide signposting, contact and support for patients as they journey through our hospital(s) as per tumour group instruction using agreed rating system.
Guide people affected by cancer to self‑management resources.

Person Specification
Qualifications
* Educated to A-level, and or NVQ Level 3 or 4 in Health and or Social Care
Experience
* Experience of working in a multi‑disciplinary team
* Experience of working in a healthcare setting
* Customer Care experience
* Knowledge of patients needs when being treated / care for in a healthcare setting
* Accurate data entry, typing and checking skills
* Ability to produce letters / reports using Word and Excel
* Understand own limitations and know where to seek advice and support.
* Have an understanding of the physical and psychological needs of a patient
* Knowledge of the elements of personalised care initiatives in cancer
* Experience of using Microsoft Office, including Word, Excel and PowerPoint
* Experience of using full range of IT systems and patient data systems
* Experience of working with patients with cancer
* Experience of handling patient complaints
* Experience of using Somerset Cancer Register
* Understanding of Primary Care, GP and community services Experience of working in the NHS
* Experience of using digital dictation systems
* Have the ability to triage to the appropriate services
