Job Summary
We are seeking a skilled and compassionate Advanced Nurse Practitioner to join our healthcare team.
Responsibilities
* Triage patient requests
* Handle on the day acute appointments
* Work alongside the Duty Doctor to handle on-the-day capacity
* Provide assessments, screening, treatment services, and health education advice
* Provide appropriate follow ups for patients
* Provide assistance to the Practice Nursing team
* Ensure accurate notes are recorded
* Attend and participate in practice meetings as required
* Assist in formulation of strategy and policy
* Undertake administrative procedures
* Participate in collection of stats for audits
Requirements
* 2+ years ANP qualification
* 2+ years primary care experience
* Ability to work to 10-minute appointments
* Experience & qualifications in chronic disease management
* Awareness of accountability of own role and other roles in a nurse-led service.
* Awareness of clinical governance issues in primary care.
* Knowledge of patient group directions and associated policy.
* Change-management skills and ability to support patients to change lifestyle.
* Good communication skills, both written and verbal.
* Ability to communicate difficult messages to patients and families.
* Negotiation and conflict management skills.
* EMIS Web clinical system experience.
* Management of long-term conditions.
* Involvement in implementing and using protocols and clinical guidelines.
* Experience of audit.
Job Type: Part-time
Pay: £27.00-£30.00 per hour
