Home Intensive Treatment Clinician

The Scarborough, Whitby and Ryedale Crisis Resolution Home Treatment Team is looking for a Registered Mental Health Clinician to join our team for a 6 month secondment period (applying candidates must have agreement from their line manager to be released from their substantive role). The team provides a responsive assessment and home treatment service to those experiencing an acute mental health crisis in the community.

As a Home-Based Treatment Clinician, you will be expected to work collaboratively with Service Users, Carers and other professionals, offering interventions as an alternative to hospital admission as the primary element of your role.

Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme.


Main duties of the job

Providing a telephone triage and support service to service users and carers who self refer via the All Age Crisis Line and supporting service users who are awaiting Mental Health Act Assessment in the Health Based Place of Safety.

To work as a member of the Crisis Resolution and Home Treatment Team, providing intensive home treatment to patients and support for their families in accordance with Trust and CRHTT Policies and Procedures.

To manage a defined caseload of clients experiencing an acute mental health episode, using evidence based, client centered, hope and recovery, health promotion and relapse prevention orientated principles and practice.

Job responsibilities

To improve the lives of people with mental ill health and/or learning disabilities by minimising the impact of their condition through the delivery of excellent services to promote recovery and wellbeing.

To facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision to community support workers and students as appropriate.

Takes steps to obtain patient consent to care and treatment in accordance with the Mental Capacity Act, ensuring that patients who lack mental capacity remain at the centre of decision-making and are fully safeguarded.

For patients requiring home treatment, develops, implements and evaluates intervention plans as part of the multi-disciplinary team, with the involvement of the patient and where appropriate, their carer.

Undertakes risk assessments in accordance with the Trusts Clinical Risk Assessment and Management Policy, devising and implementing actions and intervention plans which take the risk formulation fully into account.

To assist in identifying, developing and providing a multi-disciplinary approach within the service in accordance with the needs of services users and the requirements of the service.

To utilise a range of therapeutic interventions for adults with acute mental illness.

Working with service users in their own homes to promote maximum independence whilst experiencing mental health crisis.

To contribute to the planning and implementation of induction, training and education of students and new staff.

To participate in the preceptorship programme during the first post registration year in the NHS.

A willingness to undertake practice placement educators course and participate in student education.

To assist the senior staff by identifying personal and professional training needs in order to appropriately plan for future training and development.

To provide effective supervision, support and guidance for non-registered workers and any work delegated to them. To participate in planning of own clinical supervision.

Person Specification


Qualifications

* Relevant professional qualification required and appropriate current professional registration
* Evidence of Continuing Professional Development
* Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
* MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards. Qualification must be achieved within agreed timescale


Experience

* Working with people with mental ill-health
* Working in a person centred therapeutic organisation
* Working in a community setting
* Working collaboratively with service users and their families/carers
* Working in a multi-disciplinary team
* Working in partnership with other agencies
* Previous experience in a community mental health setting


Knowledge

* Evidenced based knowledge of best practice in caring for patients in the designated field
* Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
* Care Programme Approach and its application in practice
* Understanding of Safeguarding and its application in practice
* Clinical Risk Assessment and Management and its application in practice
* Clinical Governance and Clinical Supervision
* Research and development methodology
* Awareness of psychological models of care and treatment
* Leadership knowledge and Skills
